Pre-Procedure Information

Patient Telephone Interview
Some patients may find it helpful to call for a Patient
Telephone Interview prior to their scheduled pre-procedure
visit. By calling before your visit, your pre-procedure history
and instructions will already be completed which can shorten
the amount of time needed for your pre-procedure visit.
The Patient Telephone Interview takes approximately 15
minutes. Please call at least two to three days before your
pre-procedure visit or the day of surgery if you do not have a
pre-procedure visit scheduled.

When you call, you will need to have available all of the
medicines you are currently taking so you can read the labels
to the nurse. You may also wish to have a pen or pencil
and paper ready to write down instructions regarding your
pre-procedure diet, medications and day of procedure
information including the time you need to arrive at the
hospital on the day of your procedure.

The Patient Telephone Interview number is 770-538-7886.
A nurse is available to take calls Monday – Friday, 9 a.m. – 4
p.m. If all lines are busy during these times, please leave a
message and a nurse will return your call as soon as possible.

Hospital Interview
You may need to come to the hospital for your pre-procedure
interview. If you need to come to the hospital, your preprocedure
visits may take from one to four hours depending
on the tests your physician has ordered for you.
We will do everything possible to keep you visit moving
along smoothly. Please eat and take your scheduled
medications as you typically do. When you come for your
pre-procedure interview, please bring the items listed below:

- Current chest x-rays and/or EKG results. If you have had a chest x-ray within a year or EKG within six months, please bring a copy of the report to your pre-procedure visit or these may need to be repeated.
-All paperwork from your doctor’s office
-A list of all medications you are currently taking
-Insurance information including insurance card
-Driver’s license or other photo identification
-Social Security card
-Credit card or check for deposit (a private room deposit may be required)
-Eyeglasses (if needed to read forms)
-Advance directives (if you have them)

Other Pre-Procedure Instructions
• Call your doctor or surgeon if you think you may be getting
sick. This includes symptoms such as vomiting, fever, rash,
nausea or a cold. Your procedure may have to be postponed.
• Please call if you are delayed or there is another reason you
cannot come for your surgery. To contact the Main
Campus, call 770-531-6223; to contact the Lanier Park
Campus, call 678-343-4339. If you call after hours, call the campus where you are having your procedure and ask for the nursing supervisor. The after-hours number for the Main Campus is 770-535-3553; for the Lanier Park Campus is 678-343-4000.
