Sleep Quality Assessment

You can download a comprehensive sleep assessment questionnaire by clicking on the link below and printing out the form.  Once you complete the assessment, you can mail or fax to the Sleep Disorders Center and they will review the assessment free of charge and contact you within two weeks of receiving to let you know your results.  You can also take the results to your doctor and review your answers with him.
