Congestive Heart Failure
Support Services


Northeast Georgia Medical Center (NGMC) offers support services to patients with congestive heart failure (CHF). The purpose of the service is to provide education on the CHF disease process so the patient and family members will understand the need for their active participation in treating and preventing reoccurrence of symptoms of this illness.  

In addition to education, the CHF support service offers at-home monitoring by telephone of the patient's compliance with the course of therapy prescribed by the referring physician.  The program is set up to monitor the patient’s weight, blood pressure, fluid intake, medication compliance and adherence to dietary prescription. Bathroom scales and blood pressure monitors are provided at no cost to patients who do not have the means to measure their weight or blood pressure.

Currently, the CHF support service assists approximately 150 200 patients per month both in the inpatient setting at NGMC’s Main Campus and Lanier Park Campus as well as outpatients in their homes. CHF patients must have a physician's referral to take part in the CHF support service.
