Transitional Care Unit

The Transitional Care Center (TCC) is a center designed for short-term rehabilitation patients and patients who need additional care in order to make a successful transition from hospital to home. The center is located at New Horizons North nursing center but includes separate dining and activity areas specifically for TCC patients as well as a dedicated entrance for family members and visitors.

Typical patients include:

• Patients with fractures requiring rehab

• Patients with joint replacements

• High functioning stroke patients requiring rehab

• Recuperating surgical patients requiring rehab and/or additional nursing care

• Wound care patients

 

The care in the TCC focuses on rehabilitation with the goal of transitioning the patient to home within 30 days. Appropriate levels of rehab are provided by Northeast Georgia Medical Center’s physical thera-pists, speech pathologists and occupational therapists.
