Advance Care Planning
Advance directives are documents that direct end of life care including living wills and durable powers of attorney for healthcare. The purpose of advance directives is to state your wishes and opinions so those decisions are not left for someone else to make.

Regardless of your age, wealth or health, documenting your wishes today means your loved ones won't have to make difficult decisions later. To help patients, families and the hospitals that serve them, the American Hospital Association (AHA) has developed an educational campaign entitled “Put It in Writing” which includes the following free resources:

  • a brochure with basic facts about advance directives
  • a wallet ID card to alert healthcare workers you have advance directives
  • a link to the advance directives forms for your state
  • other helpful links and websites about advance directives
